Job summary

Tender Touch Rehab Services is seeking a licensed Physical Therapist to join our per diem team in SNF settings. You will evaluate mobility, strength, balance and function, and create personalized care plans while collaborating with a supportive interdisciplinary group.

We offer flexible Per Diem schedules, ongoing professional education, mentorship for new grads, and opportunities for advancement within Enhance Therapies family. Must hold an active PT license in the state.

Qualifications

• Current license as Physical Therapist in the state of practice.
• Degree from an APTA-accredited Physical Therapy program (BS, MS, or DPT).
• New grads welcome; mentorship available.

Responsibilities

• Evaluate and treat patients with mobility, strength, balance, and functional impairments.
• Create and implement personalized care plans.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet all regulatory requirements.
• Ensure all required physician orders are obtained and up to date.
