Responsibilities

• Consult with patients to learn about their physical condition and symptoms, and review medical history and referrals.
• Diagnose movement dysfunction by listening to patients and observing them as they move.
• Develop individualized treatment plan.
• Coach patients and teach therapeutic massage to ease pain and promote healing.
• Maintain patient records by tracking goals and progress.
• Use standardized assessments and evidence-based practice to support clinical interventions.
• Assess progress and adjust treatments in the clinical record per requirements.
• Evaluate treatment outcomes and modify services based on outcomes and referrals.
• Provide consultation to patients, families, and other providers related to physical disorders.
• Communicate with supervisor and health team regarding progress, barriers, and plans.
• Provide discharge summaries per regulatory requirements.
• Document all required information in the electronic system and maintain patient records.
• Participate in trainings, in-services and conferences.
• Able to supervise and support physical therapy assistants and aides.
• Adhere to company policies and code of conduct.
