Job summary

Proforma Health in Danville, IL seeks a licensed Physical Therapist to evaluate and treat patients of all ages, developing personalized care plans based on clinical findings, patient goals, and evidence-based practices.

You will perform assessments, design and adjust plans, educate patients and families, document care, and collaborate with the healthcare team to optimize outcomes in a supportive environment.

Qualifications

• Bachelor's Degree in Physical Therapy.
• Licensed Physical Therapist (PT) in good standing.
• BLS certification within 30 days.
• Knowledge of orthopedic and neurologic conditions, clinical examination techniques, and documentation standards.
• Strong communication, empathy, and body mechanics skills.
