Job summary

Advocate Health is hiring a Physical Therapist to evaluate patients, develop and implement individualized plans of care, and monitor progress across outpatient and acute rehabilitation settings. You will coordinate with physicians and healthcare teams to ensure optimal patient outcomes, discharge planning, and documentation in accordance with policies.

The role requires a Doctorate in Physical Therapy, state licensure, and BLS certification.

Qualifications

• Doctorate degree in Physical Therapy from an accredited school.
• Licensed Physical Therapist in the state of practice.
• BLS for Healthcare Providers certification.

Responsibilities

• Interpret physician referrals and perform musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Develop and implement plans of care with functional goals; adjust as needed based on progress.
