JOB REQUIREMENTS

Stoughton Health has an excellent opportunity for a Physical Therapist to join our Rehab Services department on a Per Diem basis.

• The Physical Therapist (PT) is responsible for the full range of physical therapy treatments and tasks.
• Evaluate and treat patients with various disabilities.
• Designs and monitors therapeutic programs to ensure safe progression toward therapeutic goals.
• This role is for Inpatient and Outpatient.

• The Physical Therapist Assistant (PTA) is responsible for the full range of physical therapy treatments and tasks that have been selected and delegated by the supervising physical therapist.

Position Requirements:

• (PT) Bachelor's Degree.
• Completion of a physical therapy course approved by the Council on Medical Education and Hospitals and the American Medical Association in collaboration with the American Physical Therapy Association.
• (PTA) Graduate of an accredited two-year college program approved by the APTA with proof of successful completion of the program.
• (PT) Licensed as a physical therapist in the State of Wisconsin or eligible for licensing.
• (PTA) Licensed as a physical therapist assistant in the State of Wisconsin or eligible for licensing.
• Current BLS certification.
• Basic computer competency skills are expected for this position.
• Microsoft Office Suite Skills: Beginning Skills: Able to perform basic things such as opening a document, change font, save a document, and write a letter.

Work Shift: Per Diem 2 Shifts per month (if shifts are available) Monday Friday
