Job summary

Care 2 You Home Health is seeking a compassionate Physical Therapist to join our team in Oakland and Macomb County, MI. You will evaluate patients, develop personalized plans, and administer therapies to improve mobility, strength, and balance.

Travel to patients’ homes is required. You will use AI-supported tools to streamline documentation and daily workflows while delivering patient-centered care with a focus on safety and independence.

Qualifications

• Licensed to practice physical therapy in the state of employment.
• Must pass criminal background and MVR checks.
• Valid state driver’s license with proof of insurance.
• Current CPR certification (hands-on CPR preferred).
• TB testing per agency policy; annual symptom screening.

Regulatory Requirements

• Licensed or registered in the State of employment
• Must pass a criminal background check & MVR check.
• Completed health statement acknowledging ability to perform the duties of the position.
• Valid state driver's license and provide proof of current automobile insurance.
• Current CPR card; On-line CPR certification accepted; Hands-on CPR certification preferred.
• TB testing per agency policy; (1 or 2 step TB skin test within 12 months of hire & annual TB symptom screening thereafter).
