Job summary

Collage Home and Community Rehabilitation Partners delivers patient-centered neurological rehabilitation in the home and community setting, serving Upland and surrounding areas. As a Physical Therapist, you will work in patients' homes with a focus on functional goals and durable outcomes, with PRN hours available.

We provide treatment for traumatic brain injuries, spinal cord injuries, CVAs, and other neuro or complex ortho diagnoses in the most functional setting there is - a person's own home and community.

We are looking for experienced Physical Therapists who want to provide truly patient-centered care in Upland, CA and surrounding areas - with PRN positions available. Our interdisciplinary teams create integrated treatment plans that are practiced in patients' homes and communities. Our therapists are creative in their approach and incorporate a variety of activities to ignite interest while addressing functional goals. This evidence-based approach results in durable outcomes.
