Collage Home and Community Rehabilitation Partners in Perris, CA is seeking experienced Physical Therapists to deliver truly patient-centered care in patients' homes and surrounding communities. PRN positions available for flexible scheduling and commitment to quality neurorehabilitation.

Therapists evaluate and treat impairments related to mobility, balance, and functional goals, collaborating with families and care teams to design individualized plans in the patient's own environment.

Qualifications

We are looking for experienced Physical Therapists who want to provide truly patient-centered care in Perris, CA and surrounding areas - with PRN positions available. Our interdisciplinary teams create integrated treatment plans that are practiced in patients' homes and communities. Our therapists are creative in their approach and incorporate a variety of activities to ignite interest while addressing functional goals. This evidence-based approach results in durable outcomes.
