Job summary

FellowshipLIFE in Monmouth/Ocean County, NJ is seeking a Full-Time Physical Therapist to join our Fellowship Therapy at Home team delivering outpatient therapy in clients' homes and community settings. You will provide high-quality care, coordinate with a multidisciplinary team, educate clients and caregivers, and travel between client locations throughout the day.

A New Jersey PT license is required, with 1 year outpatient/home health experience preferred, and a commitment to independent work.

Qualifications

• Current New Jersey Physical Therapist License (Required)
• Minimum 1 year outpatient or home health experience (Preferred)
• Strong communication and time management skills
• Ability to travel between client locations throughout the day
• Comfortable working independently

Responsibilities

• Deliver personalized, high-quality outpatient therapy in home and community settings
• Create positive, engaging experiences that empower clients to achieve their goals
• Ensure compliance with documentation and care standards
• Collaborate with a multidisciplinary team to optimize outcomes

We are seeking aFull-Time Physical Therapist to join our growing team in Monmouth & Ocean Counties.
This is a full-time position ranging from 30–40 hours per week based on caseload.
