Job summary

FOX Rehabilitation is seeking a licensed Physical Therapist for in-home and senior living care in Longbranch, NJ. You’ll deliver one-on-one treatment to older adults, perform evaluations, and create personalized plans of care within our Geriatric House Calls model.

Clinicians enjoy autonomy, flexible scheduling, and opportunities to collaborate with an interdisciplinary team. Telehealth options may be included to enhance access and continuity of care.

Qualifications

• Licensed Physical Therapist in NJ or eligible to obtain licensure
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

Responsibilities

• Provide one-on-one physical therapy to older adults in homes, private residences, or senior living communities
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions that support wellness, mobility, and functional independence
• Document effectively within FOX’s EMR system in accordance with regulatory standards
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and senior living staff to promote safety and aging in place
• Provide telehealth/virtual care when appropriate to extend access

Physical Requirements

FOX clinicians must be able to meet the physical demands ofin‑homeandcommunity‑basedcare, including:

• Exerting medium physical effort, including lifting 25–50lbsoccasionally, 10–25lbsfrequently, and up to 10lbsconstantly




• Performing CPR




• Providing moderate to maximumassistancewith patient transfers and bed mobility




• Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities




• Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model




• Wearing PPE whenrequired

In New Jersey, the standard base pay range for a Full‑Time role is $70,000 - $102,000 annually with an opportunity to earn more. Part‑Time and FOX Flex (PRN) will be pro‑rated.
