Job summary

Care2You Home Health is seeking a dedicated Home Health Physical Therapist to deliver one-on-one care in patients' homes, helping improve mobility and independence.

You will assess, treat, educate, and document progress in the EMR, collaborating with physicians and the care team to support community members.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license in the State of Michigan.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Minimum of one year of Physical Therapy experience preferred.
