Job summary

St. Luke’s Health System is seeking a Physical Therapist to join its Lifestyle Medicine team. The role emphasizes collaborative care with physicians, nurses, RD, RT, PT, and EP’s to support patients in lifestyle-focused treatment and education.

Responsibilities include standard clinic hours, multidisciplinary collaboration, and working with patients with cardiac and pulmonary diagnoses. A degree from an accredited PT program and state licensure are required.

Qualifications

• Education: Degree from an accredited physical therapy program
• Licenses/Certifications: Valid license to practice physical therapy in state of practice; Current Basic Life Support (BLS) certification.

Responsibilities

• Schedule: Monday-Friday, 7:30AM-4PM.
• Collaborate with a multi-disciplinary team.
• Work in a clinical environment with cardiac and pulmonary diagnoses.

St. Luke's Lifestyle Medicine focuses on using a collaborative care model to provide treatment through medication, coaching and education. A multidisciplinary team with Physicians, RN, RD, RT, PT, and EP's work together to help patients take control of their health through Lifestyle Medicine Including prescribing and supervised care.

• Schedule: Monday-Friday, 7:30AM-4PM
• Collaborate with a multi-disciplinary team
• Work in a clinical environment with cardiac and pulmonary diagnoses

Qualifications:
