Fox Rehabilitation in Lenoir City, TN seeks licensed Physical Therapists to provide one-on-one geriatric care in patients' homes or senior living communities. You’ll evaluate, plan, and deliver evidence-based therapy, collaborate with a multidisciplinary team, and may incorporate telehealth for flexibility and continuity of care.

The role emphasizes autonomy, meaningful patient outcomes, and the ability to create your own treatment schedule while building lasting relationships with older adults

Qualifications

• Active Physical Therapy license in TN or eligibility to obtain.
• DPT or equivalent degree from an accredited program.
• Proficient in EMR and documentation.
• Current CPR certification

Responsabilités

• Provide one-on-one physical therapy to older adults in homes and communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy to support wellness, mobility, independence.
• Document in FOX’s EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients and caregivers on safety and aging in place.
• Provide telehealth when appropriate to extend access.

Responsibilities

• Provide one-on-onephysical therapyto older adults wherever they call home (Privateresidences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)

• Perform evaluations, develop individualized plans of care, and deliverevidence-basedtreatment

• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence

• Document effectively within FOX’s EMR systemin accordance with regulatory standards

• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists

• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place

• Provide telehealth / virtual care whenappropriate toextend access and flexibility

Qualifications

• ValidPhysical Therapylicense in the state(s) of practice, or eligibility to obtain

• Degree from an accreditedPhysical Therapyprogram

Physical Requirements

FOX clinicians must be able to meet the physical demands ofin-homeandcommunity-basedcare, including:

• Exerting medium physical effort, including lifting 25–50lbsoccasionally, 10–25lbsfrequently, and up to 10lbsconstantly

• Performing CPR

• Providing moderate to maximumassistancewith patient transfers and bed mobility

• Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities

• Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model

• Wearing PPE whenrequired
