Job summary

FOX Rehabilitation is expanding in Lafayette, PA, seeking licensed Physical Therapists to deliver one-on-one care to older adults in private residences and senior living communities. The role emphasizes autonomy, flexible scheduling, and meaningful patient outcomes within the Geriatric House Calls™ model.

Clinicians may include telehealth and virtual care to extend access and continuity of care, supporting more patients while maintaining high-quality, individualized treatment plans.

Qualifications

• Licensed Physical Therapist in the state of practice or eligibility
• Degree from an ACPE-accredited Physical Therapy program
• Proficiency with EMR systems and basic computer skills
• Current CPR certification

Responsibilities

• Provide one-on-one physical therapy to older adults in their homes or senior living settings
• Perform evaluations, create individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions focusing on mobility, independence and function
• Document in the EMR per regulatory standards
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff to promote safety and aging in place
• Provide telehealth when appropriate to extend access

Responsibilities

• Provide one-on-one physical therapy to older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
• Document effectively within FOX’s EMR system in accordance with regulatory standards
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
• Provide telehealth / virtual care when appropriate to extend access and flexibility

Employment Types Available

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

Physical Requirements

• Exerting medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
• Performing CPR
• Providing moderate to maximum assistance with patient transfers and bed mobility
• Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities
• Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model
• Wearing PPE when required
