Job summary

River Valley Home Health in Arizona is seeking a dedicated Physical Therapist to design individualized care plans, deliver hands-on therapy, and collaborate with a supportive team with scheduling flexibility to fit your lifestyle.

You will assess patients in their homes, educate families on therapy techniques, and maintain thorough documentation. Active AZ PT license, CAPTE degree, 2+ years home health experience, CPR, and reliable transportation required.

Qualifications

• Must hold and maintain an active license as required by Arizona State Board of Physical Therapy.
• Degree from a CAPTE-accredited program; passed state-approved exam.
• Minimum 2 years’ experience (home health preferred).
• Current CPR certification.
• Valid driver’s license, insurance, and reliable transportation.

Responsibilities

• Develop and update individualized care plans.
• Provide physical therapy services per physician’s plan of care.
• Assess functional status, mobility, and home environment.
• Educate patients and families on therapy techniques and equipment.
• Maintain accurate documentation and communicate progress to physicians.
