FOX Rehabilitation seeks licensed Physical Therapists to provide one-on-one geriatric care in patients' homes and senior living communities across Jefferson and surrounding areas. You will evaluate patients, create individualized plans, and deliver evidence-based therapy that improves mobility and independence.

• Licensed Physical Therapist in the state(s) of practice or eligibility to obtain license.
• Degree from an accredited Physical Therapy program (PT/ DPT).
• Basic computer skills and EMR proficiency required.
• Current CPR certification.

Responsabilidades

• Deliver one-on-one physical therapy to older adults in home or community settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Conduct sessions promoting mobility, independence, and functional goals.
• Document thoroughly in FOX’s EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients and caregivers to promote safety and aging in place.

Responsibilities

• Provide one-on-onephysical therapyto older adults wherever they call home (Privateresidences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
• Perform evaluations, develop individualized plans of care, and deliverevidence-basedtreatment
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
• Document effectively within FOX’s EMR systemin accordance withregulatory standards
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
• Provide telehealth / virtual care whenappropriate toextend access and flexibility

Qualifications

• ValidPhysical Therapylicense in the state(s) of practice, or eligibility to obtain

Physical Requirements

FOX clinicians must be able to meet the physical demands ofin-homeandcommunity-basedcare, including:Exerting medium physical effort, including lifting 25–50lbsoccasionally, 10–25lbsfrequently, and up to 10lbsconstantly

• Performing CPR
• Providing moderate to maximumassistancewith patient transfers and bed mobility
• Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities
• Driving to patient homes and senior living communities as part of the Geriatric House Calls model
• Wearing PPE whenrequired
