Job summary

West Park Rehabilitation and Nursing Center is seeking a dedicated Physical Therapist to join our interdisciplinary team. In this role you will evaluate patients, develop and adjust treatment plans, and provide exercise, manual therapy, and education to residents.

Qualifications

• Evaluate and assess patients' physical abilities and develop personalized treatment plans to enhance independence and well-being.
• Implement and modify treatment plans to meet changing patient needs and goals.
• Provide physical therapy interventions, including exercise, manual therapy, and education to patients and their caregivers.
• Collaborate with other healthcare professionals to ensure comprehensive care and achieve optimal outcomes.
• Document patient progress and maintain accurate records.
• Partake in staff meetings and contribute to the development of new programs and services to improve patient care.
