Job summary

Evolve Therapy Services is seeking a dedicated Physical Therapist for an in-house Skilled Nursing Facility in South Sioux City, NE. The role supports comprehensive PT services with flexible scheduling options, including Full Time, Part Time, or PRN.

In addition to compassionate patient care, the position offers a $10,000 sign-on or relocation bonus, competitive pay, and opportunities for growth within the organization. Licensure requirements and ongoing CEUs are expected.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and department procedures.
• Adhere to documentation and billing procedures for PT services.
• Develop treatment plans with short- and long-term goals and communicate findings to residents and families.
• Complete CMS-compliant documentation in NetHealth EMR.
• Conduct screenings and evaluations to determine need for intervention and treatment.
