Brookside Healthcare and Rehabilitation Center in Roslyn, PA is seeking a compassionate Physical Therapist to join our in-house rehabilitation team serving short-term rehab, long-term care, and residents. You will evaluate residents, develop care plans, and provide direct treatment with a focus on quality outcomes.

The role emphasizes compassionate care, adherence to Medicare regulations, and collaboration with staff to drive patient progress and program development.

Qualifikationen

• Graduated from an accredited program approved by your professional association.
• Current licensure by the applicable state agency.
• Clinical experience in a skilled nursing facility is preferred.

Aufgaben

• Evaluate residents and develop appropriate care plans.
• Monitor residents' progress and adjust treatments.
• Provide direct resident treatment per plan of care, including group therapy.
• Document services in NetHealth on a timely basis.
• Demonstrate knowledge of Medicare and reimbursement regulations.
• Exhibit courtesy, compassion, and respect to residents, families, caregivers, visitors, physicians, administrators, and co-workers in each interaction.
• Participate in clinical program development and implementation.
• Contribute to team meetings and decision-making processes as defined by the program.
• Responsible for service and operational excellence of all assigned activities to ensure quality outcomes.
• Participate in quality improvement activities as needed.
• May perform additional assigned administrative duties.
