Job summary

Safe Life Home Health Care is seeking a licensed Physical Therapist to provide patient-centered therapy services in the Elgin area and surrounding Cook and Kane counties in Illinois. You will supervise assistants and implement the plan of care with adherence to agency policies.

The role requires travel to patients' homes, strong communication with families, and timely documentation. A minimum of one year in home health care is preferred, along with CPR and TB testing.

Qualifications

• Must be licensed Physical Therapist by the state of Illinois.
• Minimum one year of Home Health Care clinical experience.
• Must have a car with valid insurance.
• Annual TB test and CPR certification required.
• Able to read and interpret instructions related to patient care.
• Able to travel to prospective patient's place of residence.

Responsibilities

• Performs initial evaluations within 48 hours of referral and re-evaluations every 30 days, coordinating plan of care with the physician as needed.
• Prepares clinical and progress notes and required summaries, as appropriate.

• Instructs patient and/or family in the use of equipment, safe patient management, and home therapy programs as needed.
• Coordinates care with agency DON and other staff as appropriate.

Position Overview

Provides Physical Therapy services to patients as directed by the plan of care in line with the philosophy, objectives, and policies of the agency. Supervises Physical Therapy Assistants and maintains responsibility for the implementation and follow-through of PT procedures.

Responsibilities

• Prepares clinical and progress notes and required summaries, as appropriate.
• Revises plan of treatment and contacts the physician for change in plan of care.
• Advises and consults with patient's family and other health care staff.
• Coordinates care with agency DON and other staff as appropriate.
• Observes, records, and reports any changes in the patient's condition and updates the agency on changes in the patient's status or schedule changes.
• Supervises the Physical Therapy Assistant.
• Instructs patient and/or family in the use of equipment, safe patient management, and home therapy programs as needed.
• Communicates effectively with patients, caregivers, and families as well as agency staff.
• Provides quality and appropriate care and reports issues or concerns to the DON/Administrator.
• Assures that all tasks are performed in a timely manner.
• Checks in on Physical Therapy Assistants every 4-6 patient visits.

Qualification

• Must be a licensed Physical Therapist by the state of Illinois.
• Must have at least one year of Home Health Care clinical experience.
• Car with required proof of insurance coverage.
• Maintains annual TB test, CPR certification, and other required.
• Able to read and interpret instructions related to patient care.
• Able to effectively perform and prioritize multiple functions or tasks as needed.
• Must be able to travel to prospective patient's place of residence.
