Job summary

Metro Physical & Aquatic Therapy is seeking motivated Physical Therapists for home-based care across Nassau County. This per diem, part-time role offers schedule flexibility, autonomy, and meaningful patient relationships as you deliver high-quality therapy in patients’ homes and community settings.

Requirements include New York licensure or eligibility, and graduation from an accredited Physical Therapy program. You should be able to travel to patients’ homes within the geographic territory.

Qualifications

• Graduate of Physical Therapist program.
• Licensed PT in New York or eligible.
• Home health experience preferred but not required.
• Ability to travel within the geographic territory.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans.
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family.
• Plan, prepare and implement customized therapy programs to restore function and promote independence.
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques.
• Provide timely and accurate documentation for each visit.
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning.
• Deliver care in compliance with agency policies and professional practice standards.

The anticipated base hourly range for this position is $70-90. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography.
