Job summary

Eddy Visiting Nurse & Rehab Association is seeking a Homecare Physical Therapist to manage a home-based caseload in the Capital Region. The role offers competitive pay ($43.73–$56.58/hour) and a $5,000 sign-on bonus, with a focus on independent practice and patient-centered care.

Eligible candidates will hold a NYS PT license, have at least one year of PT experience, and may have home care experience. Schedule typically 8–4 with weekend rotation; benefits and orientation provided.

Qualifications

• Graduate of an approved Physical Therapy program with a Bachelor of Science, Master’s or DPT degree.
• Current NYS PT license and registration.
• One year PT experience required. Home care experience preferred.

Responsibilities

• Provide physical therapy services in patients' homes to maximize functioning.
• Evaluate and treat patients with neurologic, orthopedic and medical conditions.
• Educate patients and caregivers to promote independence and self-management.
• Deliver care with a focus on whole-patient outcomes.
• Work schedule typically 8:00–16:00 with some variation.
• Weekend rotation every 6 weeks with one holiday per year.

Eddy Visiting Nurse & Rehab Association has an exciting opportunity for an Physical Therapist in a designated area of our Capital District responsible to manage a caseload of patients requiring home-based therapy, primarily post acute or subacute.

What you will do

• Promotes/provides physical therapy services in the home to assist the patient in achieving his/her optimal level of functioning.

• Evaluates and treats individuals with functional deficits secondary to neurologic, orthopedic and medical conditions.

• Teach patients/caregivers and help them to become independent with their treatments and self-management of their chronic illness(es)

• Work schedule is generally 8-4 during the week. Flexible work schedule.

• Weekend rotation: Every 6 weeks and one holiday per year

What you will need

• Graduate of an approved Physical Therapy program with a Bachelor of Science, Master’s degree or Clinical Doctorate of physical therapy degree

• Current NYS PT license and registration

• One year PT experience required

• Home care experience preferred, but not required

Pay Range: $43.73-$56.58 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
