Job summary

Enhabit Home Health & Hospice is seeking a Physical Therapist to examine and treat patients with physical impairments in the home setting, using modalities to improve mobility and function. You will administer skilled care per physician orders and the established plan of care under supervision of the Branch Director.

You will educate families and the health care team, supporting rehabilitation at home and participating in Specialty Programs such as Balance and Fall Prevention, Spine Safety, and

Qualifications

• Must possess a valid state driver's license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed in the state of employment.

Responsibilities

• Examine and treat patients with physical impairments using modalities to improve mobility and function.
• Deliver skilled care per physician orders and plan of care under Branch Director supervision.
• Educate family and health care team to support at-home rehabilitation.
• Participate in Specialty Programs such as Balance and Fall Prevention, Spine Safety, and joint replacement programs.
