Job summary

BAYADA Home Health Care is seeking a Physical Therapist to perform home health visits on a full-time basis, with options for part-time or per diem as needed. The role covers adult and geriatric clients in Maryland.

Responsibilities include evaluating mobility, designing individualized therapy plans, and coordinating with nursing staff. A Maryland PT license and EMR familiarity are required.

Qualifications

• Current Maryland Physical Therapist license.
• Minimum of one year of PT experience.
• Graduation from an APTA-approved program (DPT/MS).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational and problem-solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements of mobility, strength, balance, and safety.
• Implement protective interventions to prevent wounds and promote healing.
• Develop and implement individualized care plans (exercises, gait training, balance).
• Continuously assess and revise the PT care plan with nursing in the multidisciplinary team.
• Educate clients and families in rehabilitative care and safety.
• Accurately document observations and interventions in EMR.

BAYADA Home Health Care is currently seeking an experienced Physical Therapist (PT) for a Full-Time, Part-Time or Per Diem opportunity performing home health visits. This office services adult and geriatric clients on a per visit basis in territories throughout Carroll County, Maryland.

Must be available to see patients on a full-time, part-time or per diem basis between the hours of 8:30a-5p Monday-Friday!

Responsibilities for a PT - Physical Therapist include:

Make home visits to clients in designated geographic territories.

• Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
• Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents.
• Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
• Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client’s health, safety, and independent living.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.

Qualificationsfor a PT - Physical Therapist include:

• A current Maryland Physical Therapist - PT license.
• A minimum of one year of recent work experience as a Physical Therapist - PT.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Additional Information:

Base Pay, depending upon qualifications: $88,920-$120,000 annual earning potential (full time)
