Job summary

Careage Home Health seeks a Physical Therapist for a temporary travel assignment across rural South King County locations (Kent, Auburn, Federal Way, Des Moines, Covington). The role runs 13-16 weeks, part-time 16-24 hours weekly, at $85.00 per hour with travel time reimbursed per IRS rates.

Eligible candidates will be licensed in Washington, graduate from an APTA-approved program, and ideally have 2 years of PT experience and WA driver’s license to support site visits.

Qualifications

• Graduate from a physical therapist curriculum approved by the American Physical Therapy Association.
• Currently licensed as a physical therapist in the state of Washington.
• Two years' experience as a physical therapist, preferred.
• WA driver’s license and ability to travel between locations.

Responsibilities

• Provide physician orders and plan of care under supervision.
• Work with home care personnel in patient care planning.
• Direct and supervise PT Assistant and Home Health Aide as assigned.
• Take initial patient history and perform initial evaluation.
• Consult with physicians regarding changes in treatment.
• Instruct patients and families in home programs and activities of daily living.
• Complete and submit OASIS assessments and related documents.
• Utilize ICD-10 codes in documentation.

• Direct and supervise Physical Therapy Assistant and Home Health Aide, as assigned.
• Take initial patient history and make initial evaluation.
• Consult with physicians regarding change in treatment.
• Instruct patients and family in home programs and activities of daily living.
• Participates in Quality Assessment and Performance Improvement activities as assigned.
• Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition.
• Appropriately utilizes ICD-10 codes.

Requirements

• Graduate from a physical therapist curriculum approved by the American Physical Therapy Association.
• Currently licensed as a physical therapist in the state of Washington.
• Two (2) years' experience as a physical therapist, preferred.
• Home Health experience, preferred.
• Detail oriented, self motivated and capable of efficient time management.
• Critical thinker capable of working collaboratively with a team.
• Valid WA State driver's license.
