Careage Home Health is seeking a Physical Therapist to join our team. This role provides one-on-one home care across multiple locations in the Redmond area, with flexible scheduling and no on-call or weekend requirements. We offer a competitive hourly rate and travel time documentation.

This position supports ongoing professional development with no-cost continuing education and licensure renewal reimbursement, plus a comprehensive benefits package for full-time staff.

Qualifications

• Licensed physical therapist in Washington state (WA).
• Two years’ experience in physical therapy preferred, home health a plus.
• Detail oriented with strong time management and teamwork.

Responsibilities

• Provide physician orders and plan of care under the Director of Clinical Services.
• Collaborate with home health team for patient care planning.
• Direct and supervise PT Assistants and Home Health Aides as assigned.
• Take initial patient history and perform initial evaluation.
• Consult with physicians regarding treatment changes.
• Instruct patients and families in home programs and activities of daily living.
• Participate in Quality Assessment and Performance Improvement activities.
• Complete and submit OASIS assessments, reassessments, transfers, and discharges.
• Utilize ICD-10 codes appropriately.

Summary Of Responsibilities

• Direct and supervise Physical Therapy Assistant and Home Health Aide, as assigned.
• Take initial patient history and make initial evaluation.
• Consult with physicians regarding change in treatment.
• Instruct patients and family in home programs and activities of daily living.
• Participates in Quality Assessment and Performance Improvement activities as assigned.
• Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition.
• Appropriately utilizes ICD-10 codes.

Requirements

• Graduate from a physical therapist curriculum approved by the American Physical Therapy Association.
• Currently licensed as a physical therapist in the state of Washington.
• Two (2) years' experience as a physical therapist, preferred.
• Home Health experience, preferred.
• Detail oriented, self motivated and capable of efficient time management.
• Critical thinker capable of working collaboratively with a team.
• Valid WA State driver's license.
