Job summary

VitalCaring Group is searching for a Home Health Physical Therapist to join our team. This role allows you to make a significant impact on your patients' recovery and independence within the comfort of their homes.

You will evaluate patients, develop tailored care plans, and collaborate with an interdisciplinary team. The position offers flexible scheduling, a supportive work environment, and opportunities for career advancement.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current PT license in the state of practice.
• Minimum 1–2 years of home health experience preferred.
• Strong clinical assessment and critical thinking skills required.
• Excellent communication and patient education abilities.
• Valid driver's license, reliable transportation, and auto insurance.

Responsibilities

• Evaluate and assess patients’ functional status and mobility.
• Develop and implement individualized care plans.
• Provide hands-on therapy to improve patients' independence.
• Monitor patient progress and adjust treatment plans as needed.
• Document patient care and progress accurately in the EMR.
• Supervise Physical Therapist Assistants as per state guidelines.

What You’ll Do

Deliver Patient-Centered Care

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential


• Develop and implement individualized care plans in collaboration with physicians and the care team


• Provide hands‑on therapy to improve mobility, strength, balance, and independence


• Educate patients and caregivers on treatment plans, exercises, and safety strategies

Drive Outcomes & Recovery

• Monitor patient progress and adjust treatment plans as needed


• Support safe transitions of care and contribute to discharge planning


• Identify additional patient needs and collaborate with interdisciplinary teams


• Partner closely with nurses, therapists, and physicians to ensure coordinated care


• Document patient care and progress accurately and timely within the EMR


• Participate in case conferences and team meetings

Lead & Support

• Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines


• Provide guidance and education to caregivers and support staff

What You Bring

• Graduate of an accredited Physical Therapy program (DPT or Master’s)


• Current PT license in the state of practice


• Minimum 1–2 years of experience (home health experience preferred)


• Strong clinical assessment and critical thinking skills


• Excellent communication and patient education abilities


• Current CPR certification


• Valid driver’s license, reliable transportation, and auto insurance
