Job summary

Omega Home Health Care Services, Inc. is seeking a compassionate Physical Therapist to evaluate patients and provide therapy services as prescribed by physicians in the home health setting.

You will guide patients through therapeutic activities, document progress, train families on devices, and collaborate with physicians and nursing staff to plan care and discharge. Applicants must be licensed in California, CPR certified, and have a driver's license and auto insurance; institutional experience

Qualifications

• Licensed or registered by the state of California.
• Preference of at least one year of institutional experience.

Responsibilities

• Evaluate patients and provide physical therapy services as prescribed.
• Guide patients in therapeutic activities to relieve pain and restore function.
• Record and report patient responses to therapy to physicians and staff.
• Train patients and caregivers in the use of assistive devices.
• Instruct patients in the total physical therapy program and provide handouts.
• Prepare assessment, plan of care, and progress notes; integrate into patient record.
• Prepare periodic evaluations and discharge summaries.
• Follow the Plan of Care and communicate with care team.
• Notify agency of absences affecting services.
• Participate in discharge planning with staff and physicians.

The Physical Therapist is a professional who evaluates and provides physical therapy services to patients as prescribed by the attending physician.

Qualifications

• Graduate of a physical therapy school approved by:
• The American Physical Therapy Association; or
• The Council of Medication Education of the A.M.A. in collaboration with the American Physical Therapy Association; or
• The Council of Medical Education of Hospitals for the American Medical Association
• Licensed or registered by the state of California.
• Preference of at least one (1) year of institutional experience
• Valid CPR certification, driver's license, and auto insurance

Responsibilities

• Partner with the physician in evaluating patients by applying diagnostic and prognostic muscle, nerve, joint and functional ability tests, and in establishing goals based on the client’s current level of functioning and potential for improvement.
• Guide and instruct patients in prescribed therapeutic activities that are directed towards relieving pain and/or developing or restoring function to maintain maximum performance.
• Record and report to the physician and home health office staff, the patient’s reaction to the therapy program and/or any changes in the patient’s condition.
• Train patients, families, and/or caregivers in the proper use of prosthetic, orthotic, and assistive devices (canes, walkers, wheelchairs, etc.).
• Instruct patients, families, and/or caregivers in the client’s total physical therapy program (present them with handouts and instructional content).
• Prepare an assessment of the patient’s condition, develop a plan of care and a problem list, write clinical/progress notes, and incorporate everything into the patient’s record.
• Prepare periodic evaluations and summary reports of the patient’s condition.
• Follow the Plan of Care
• Communicate effectively with all those providing care to the patient.
• Report to the physician, Director of Nursing and designated Nursing Supervisor.
• Notify the Home Health Agency in a timely fashion, of absences due to illness, emergency leave, normal vacation periods, and/or special professional meetings which will affect agreed upon services with the patient/agency.
• Participate with staff, patients, patients’ families, and physicians in discharge planning activities and complete the physical therapy discharge summary within the designated framework of Agency policies.

Functional/Special Requirements

• Must be able to read 12-point font or larger type.
• Must be able to hear and speak in a manner understood by most people.
• Must be able to stoop and bend.
• Must be able to carry equipment weighing up to 10 pounds up and down stairs.
• Must be able to lift and carry effectively to be able to perform the above-listed functions.
• Must be able to travel to prospective clients’ residences.

This position is paid by the visit. Compensation for routine visits ranges from $65.00-$70.00 per visit; evaluations range from $80.00-$100.00 per visit; and admissions range from $120-$140 per visit. Actual pay rates vary depending on factors including type of visit, type of experience, location, and years of experience.
