Job summary

VitalCaring is seeking a Home Health Physical Therapist to deliver patient-centered care in patients\' homes. You will evaluate mobility, develop individualized plans, and provide hands-on therapy while educating families and coordinating with the care team.

You will monitor progress, adjust plans, and ensure safe transitions of care. This role emphasizes autonomy, compassionate communication, and collaboration with nurses, physicians, and PTAs.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master\u2019s).
• Current PT license in the state of practice.
• 12 years of home health experience preferred.
• Excellent communication and patient education abilities.
• Valid driver\u2019s license and reliable transportation.

Responsibilities

• Evaluate and assess patients\u2019 functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Support safe transitions of care and contribute to discharge planning.
• Identify additional patient needs and collaborate with interdisciplinary teams.
• Document patient care and progress accurately and timely within the EMR.
• Participate in case conferences and team meetings.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines.
• Provide guidance and education to caregivers and support staff.

What You'll Do

Deliver Patient-Centered Care

• Partner closely with nurses, therapists, and physicians to ensure coordinated care
• Document patient care and progress accurately and timely within the EMR
• Participate in case conferences and team meetings

Lead & Support

• Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines
• Provide guidance and education to caregivers and support staff

What You Bring

• Graduate of an accredited Physical Therapy program (DPT or Master's)
• Current PT license in the state of practice
• Minimum 1-2 years of experience (home health experience preferred)
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Current CPR certification
• Valid driver’s license, reliable transportation, and auto insurance
