Job summary

VitalCaring Group seeks a Home Health Physical Therapist to deliver patient-centered care in patients' homes across Clearwater, FL. You will evaluate functional status, develop individualized plans, and supervise PTAs while collaborating with nurses and physicians to ensure coordinated care.

You will educate patients and caregivers, monitor progress, and support safe transitions of care. A Florida PT license, CPR, and valid driver’s license are required.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in Florida.
• Minimum 1–2 years of home health experience preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Deliver patient-centered care by evaluating and addressing functional status, mobility, strength and rehabilitation potential.
• Develop and implement individualized care plans with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance and independence.

What You’ll Do

Deliver Patient-Centered Care

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential
• Develop and implement individualized care plans in collaboration with physicians and the care team
• Provide hands-on therapy to improve mobility, strength, balance, and independence
• Educate patients and caregivers on treatment plans, exercises, and safety strategies

Drive Outcomes & Recovery

• Monitor patient progress and adjust treatment plans as needed
• Support safe transitions of care and contribute to discharge planning
• Identify additional patient needs and collaborate with interdisciplinary teams

Collaborate & Communicate

• Partner closely with nurses, therapists, and physicians to ensure coordinated care
• Document patient care and progress accurately and timely within the EMR
• Participate in case conferences and team meetings

Lead & Support

• Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines
• Provide guidance and education to caregivers and support staff

What You Bring

• Graduate of an accredited Physical Therapy program (DPT or Master’s)
• Current PT license in the state of practice
• Minimum 1–2 years of experience (home health experience preferred)
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Current CPR certification
• Valid driver’s license, reliable transportation, and auto insurance
