Acclaim Home Health Care in Worcester, MA is seeking a skilled Physical Therapist to join our growing team. As a home health PT, you'll evaluate and treat patients in their homes, aiming to improve mobility, strength, and independence while supporting safe, personalized care.

Qualifications

• Massachusetts PT license required.
• Home health or outpatient experience preferred.
• Ability to work independently and manage a patient caseload.

Responsibilities

• Conduct comprehensive physical therapy evaluations and develop individualized treatment plans.
• Provide interventions to improve strength, mobility, balance, and independence.
• Help patients safely achieve mobility goals in their home environment.
• Educate patients and caregivers on exercises, mobility strategies, fall prevention, and equipment use.
• Coordinate care with families, physicians, and interdisciplinary team members.
• Monitor progress and adjust treatment plans as needed.
• Document clinical notes timely and accurately.

What You'll Do:

• Conduct comprehensive physical therapy evaluations and develop individualized treatment plans.


• Provide therapeutic interventions to improve strength, mobility, balance, coordination, and functional independence.


• Help patients safely achieve their mobility and functional goals in their home environment.


• Educate patients and caregivers on exercises, mobility strategies, fall prevention, and appropriate use of equipment.


• Collaborate with patients, families, physicians, and interdisciplinary team members to coordinate care and achieve optimal outcomes.


• Monitor patient progress and adjust treatment plans as needed.


• Complete timely and accurate clinical documentation in accordance with company and regulatory requirements.


• Maintain current clinical knowledge through continuing education and professional development.

What We're Looking For:

• Current Massachusetts Physical Therapist license.


• Home health or outpatient experience preferred.


• Strong clinical assessment and treatment skills.


• Excellent communication and interpersonal skills.


• Ability to work independently and effectively manage a patient caseload.


• Strong organizational and time-management skills.


• Valid driver's license and reliable transportation.
