AccentCare is seeking a Physical Therapist for Home Health in Texas. The role covers Tyler area and surrounding communities, with a focus on evaluation, treatment planning, and patient education.

This full-time, on-site position requires travel to multiple locations and adherence to agency guidelines. The ideal candidate holds a CAPTE-accredited PT degree, has at least nine months of PT experience, a valid Texas PT license, CPR certification, and a valid driver’s license.

Qualifications

  • Master's degree in physical therapy from a CAPTE-accredited program.
  • Minimum nine months experience as a physical therapist.
  • Preferred community/home health experience.
  • Licensed to practice as a physical therapist in the state of operation.
  • Must possess and maintain CPR certification.
  • Must be a licensed driver able to travel to all business locations.

Responsibilities

  • Provides patient care and documents treatment per physician orders and policies.
  • Performs patient evaluations and selects diagnostic materials.
  • Informs physician of assessment results and recommended interventions.
  • Develops and documents treatment plans with priorities.
  • Educates and counsels patients and families during treatment sessions.
  • Presents patient information to physicians and updates the team on status.
  • Provides treatment within agency and payer guidelines.
  • Sets measurable goals in collaboration with the team and patients.
  • Meets productivity standards and manages time efficiently.

Salary: $95,400 - $116,000 annually based on experience

This position is paid on a per-point basis. The compensation reflected on this posting, is an estimate of annual compensation.

What You Need to Know

The Physical Therapist is responsible for providing evaluation and treatment to patients, functioning as a member of the interdisciplinary team, providing patient and family education and counseling regarding concerns, planning, implementing and modifying treatment procedures, maintaining appropriate documentation according to policies and procedures and current standards of practice. This position manages a caseload in accordance with agency productivity standards, attends and participates in patient and other conferences and agency in-services as assigned.

Responsibilities:

As a Physical Therapist, you will:

  • Provides patient care and documents treatment in accordance with physician orders, agency policy and patient needs.
  • Performs patient evaluations in accordance with agency policy and procedure. Selects appropriate diagnostic material to assess all aspects of patient care.
  • Informs the physician of the results of the assessment and recommended clinical interventions prior to initiating care.
  • Develops and documents appropriate treatment plans for patients including selecting appropriate intervention approaches, strategies, materials and establishing treatment priorities.
  • Provides an organized and efficient patient treatment which includes advising, educating and counseling the patient.
  • Presents pertinent patient information to physicians and keeps team members informed of patient status and treatment plan.
  • Provides appropriate treatment to patients according to patient needs and within the parameters set by the agency and payer.
  • Determines appropriate goals, which are objective and measurable in nature, in collaboration with team members and patients.
  • Meets agency productivity standards and uses work hours efficiently.
