Job summary

CenterWell Home Health is seeking a dedicated Physical Therapist to deliver in-home therapy, promoting functional recovery for patients in Maryland. You will work under the Clinical Manager to develop personalized treatment plans, measure progress, and coordinate with care teams and families to optimize outcomes.

Responsibilities include administering prescribed PT, documenting progress, supervising PTAs and aides, and ensuring timely communication with physicians.

• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report patient progress to the physician, Clinical Manager, care team, patient, and family; submit evaluations, plans, progress notes, and discharge summaries.

As a Home Health Physical Therapist, You Will

• Plan and administer prescribed physical therapy treatment for patients, working onsite in their homes, to attain highest level of physical function.
• Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
• Develop and implement a rehabilitation program consistent with physician's Plan of Treatment and the goals of the patient team.
• Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.

Preferred Qualifications

Pay Range

• $65.00 - $91.00 pay per visit/unit
• $102,300 - $140,800 per year base pay

TB Statement

This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Driving Statement

This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
