Job summary

Sonder Healthcare is seeking a compassionate and motivated Physical Therapist to join our growing team delivering in-home, patient-centered care across Sacramento and surrounding areas. This Full-Time role involves initial visits, assessments, and implementing treatment plans while collaborating with PTAs and the interdisciplinary team.

You must hold an active California Physical Therapy license, have a valid driver's license and CPR/BCLS, and be comfortable traveling to patient homes.

Qualifications

• Active Physical Therapist license in the state of employment.
• Current CPR/BCLS certification.
• Home health experience preferred but not required.

Responsibilities

• Conduct initial patient visits, including comprehensive assessments and evaluations.
• Develop and implement individualized treatment plans.
• Supervise and collaborate with Physical Therapist Assistants (PTAs).
• Coordinate care with interdisciplinary team members.
