Job summary

CHI Franciscan Home Health is looking for a Weekend Start of Care Physical Therapist to cover Saturdays through Monday with 10-hour shifts across South King, Pierce, and Kitsap Counties. This home-based role focuses on improving functional movement in patients’ homes.

You will evaluate patients, administer therapy, educate families, monitor progress, and keep compliant records. Licensure and CPR/BLS are required; a driver’s license is essential.

Qualifications

• Bachelor's degree in Physical Therapy and current PT license.
• CPR/BLS certification is required.

Responsibilities

• Evaluate patients’ physical abilities, mobility, strength, and functional limitations.
• Implement therapeutic exercises, manual therapy, and modalities to improve function and mobility.
• Educate patients and families on exercises, safety, and lifestyle changes.
• Monitor progress and adjust treatment plans to achieve outcomes.
• Maintain accurate patient records in compliance with regulations.

Job Summary and Responsibilities

Now Hiring a Weekend Start of Care Physical Therapist Saturday - Monday-10 hour shifts, rotating coverage in South King, Pierce, and Kitsap Counties.

We are seeking a skilled and compassionate Physical Therapist to join the CHI Franciscan Home Health team. The successful candidate will provide physical therapy services to patients in their homes, promoting rehabilitation, recovery, and improved quality of life. As a key member of our interdisciplinary team, you will work closely with patients, families, caregivers, and other healthcare professionals to develop and implement personalized treatment plans that address each patient's unique needs and goals.

The focus of Physical Therapy is to improve functional movement. Practicing in the home setting, where patients spend most of their time, is the most "functional" place to make an impact on this!

• Conduct evaluations of patients’ physical abilities, mobility, strength, and functional limitations.
• Implement therapeutic exercises, manual therapy techniques, and modalities to improve patients’ physical function and mobility.
• Educate patients and their families about exercises, safety, and lifestyle modifications to promote recovery and prevent re-injury.
• Monitor and document patient progress, adjusting treatment plans as necessary to achieve optimal outcomes.
• Maintain accurate and up-to-date patient records in compliance with state regulations and agency policies.

Job Requirements

• Bachelor's degree from an accredited school approved by the American Physical Therapy Association
• Active registered PT license in the state which you practice
• Must obtain and maintain current CPR/BLS Certification
• One-year physical therapy experience in teaching or clinical setting is preferred, but not required.
• Current Drivers License
• Comply with federal, state and local laws and regulations

Pay Range

$47.41 - $78.23 /hour
