Pride Health is actively hiring a Physical Therapist (PT) to join our client's growing Home Health team in Centralia, WA. The Local Pay is $55.00 - $70.00/hour (W-2) with a travel pay package up to $2,800/week gross and a weekly hour guarantee of 32 hours.

Must have a PT license and at least 1 year of home health experience. You will perform SOCs, initial evaluations, routine revisits, re-evaluations and discharge assessments, delivering evidence-based therapy and documenting progress in

• Location: Centralia, WA, 98531
• Weekly Hour Guarantee: 32
• Productivity Expectations (points): 30 pts
• Coverage Areas: Centralia and surrounding areas
• Shift schedule (days, start & end times): days
• On Call/Weekend Requirement: no
• Will Milage be reimbursed between visits, from first patient to last patient (Y/N)? yes, pt to pt, not home to first or last pt

Key Responsibilities

• Conduct comprehensive home health physical therapy services, including Start of Care (SOC), initial evaluations, routine revisits, re-evaluations, and discharge assessments, while developing individualized treatment plans, improving patients' functional mobility, and meeting a productivity expectation of 30 points.
• Deliver evidence-based therapy interventions, monitor and document patient progress accurately in Homecare Homebase (HCHB)/PointCare, collaborate with physicians and the interdisciplinary team, educate patients and caregivers, and ensure compliance with Medicare, agency, and home health documentation standards across the assigned coverage areas.
