Job summary

You will document progress, teach patients and families, and coordinate with the healthcare team to ensure high-quality care across home care protocols.

The role requires a Bachelor’s degree in Physical Therapy, 1-2 years of clinical experience, and current PT licensure for DC/MD/VA, plus CPR certification.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited school.
• 1-2 years clinical experience required.
• PT license to practice Physical Therapy in DC/MD/VA required; CPR certification.
• Driver's license in good standing required.

Responsibilities

• Analyzes and documents patient responses to interventions and progress toward goals.
• Educates patients and families; communicates with the care team and schedules visits.
• Adheres to department policies and completes mandatory trainings.
• Implements and documents plans of care and adjusts as needed.
• Maintains clinical records and prepares reports with high accuracy.
