Job summary

Stratford VNA seeks a Home Care Physical Therapist to deliver in-home therapy across Stratford and surrounding communities. You’ll assess function, craft individualized plans, and guide patients toward greater independence in their own homes.

Role offers full-time, part-time, and per diem opportunities with competitive compensation, company car use, and benefits; CT licensure and at least one year of PT experience are preferred.

Qualifications

• Connecticut PT license current and in good standing.
• At least one year of physical therapy experience.
• Strong assessment, clinical, organizational, and communication skills.

In this role, you will provide home safety evaluations, assess the patient's current and prior level of functioning, develop an individualized treatment plan, and guide the patient towards greater independence and improved health outcomes. Treat a diverse patient population including post-operative joint replacements, neurological, cardiac, respiratory, renal, and oncology patients. Coordinate with physicians and interdisciplinary team members, educate patients and families, and help individuals achieve their goals. Full-Time, Part-Time and Per Diem physical therapy positions available.
