Job summary

Synchrony Rehab at Ontario Estates is seeking a Physical Therapist to provide a full range of PT services, from assessment to intervention, in an interdisciplinary setting. The role emphasizes treatment planning, patient education, and collaboration with the care team.

The position requires a state PT license and a CPR certification is preferred. Prior experience is not required beyond 0-1 years of clinical exposure, and the center offers orientation and ongoing support.

Qualifications

• Must have a current, valid state PT licensure or certification

• Education: Degree in Physical Therapy; Experience: 0-1 years

Responsibilities

• Conduct thorough assessments to evaluate patients’ physical conditions and rehabilitation needs.
• Develop and implement personalized treatment plans including exercises, manual therapy and modalities.
• Educate patients and families on exercises, posture, body mechanics and strategies to manage conditions.
• Maintain accurate documentation of progress, plans and outcomes per facility policies.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.
