Job summary

FOX Rehabilitation is seeking licensed Physical Therapists in New Jersey to deliver one-on-one geriatric care across private residences and senior living communities under the Geriatric House Calls model. Qualifications

• Licensed Physical Therapist in NJ or eligible to obtain license.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living communities.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to improve mobility, independence, and functional skills.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.

This position is partially staffed in one of our senior living community partners, Felician Sisters Convent in Lodi.

Responsibilities

• Provide one-on-onephysical therapyto older adults wherever they call home (Privateresidences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)

• Perform evaluations, develop individualized plans of care, and deliverevidence-basedtreatment

• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence

• Document effectively within FOX’s EMR systemin accordance withregulatory standards

• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists

• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place

• Provide telehealth / virtual care whenappropriate toextend access and flexibility

Qualifications

• ValidPhysical Therapylicense in the state(s) of practice, or eligibility to obtain

• Degree from an accreditedPhysical Therapyprogram

• Basic computer and EMRproficiency

• Current CPR certification

Physical Requirements

FOX clinicians must be able to meet the physical demands ofin-homeandcommunity-basedcare, including:

• Exerting medium physical effort, including lifting 25–50lbsoccasionally, 10–25lbsfrequently, and up to 10lbsconstantly

• Performing CPR

• Providing moderate to maximumassistancewith patient transfers and bed mobility

• Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities

• Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model

• Wearing PPE whenrequired

In New Jersey, the standard base pay range for a Full-Time role is $70,000 - $102,000 annually with an opportunity to earn more. Part-Time and FOX Flex (PRN) will be pro-rated.
