Qualifications

• Licensed Physical Therapist in NY or eligible.
• Ability to evaluate a patient’s need for physical therapy and develop a treatment plan.
• Provide appropriate care and treatment to patients per physician plan and standards of care.
• Experience instructing family members, PTAs, Home Health Aides, and other caregivers.
• Mandatory Certifications: First Aid, AED, & CPR.

Responsibilities

• Evaluate patients and determine PT needs.
• Develop and implement personalized treatment plans.
• Educate and coach caregivers and family members in PT techniques.
• Collaborate with clinicians and other staff to optimize patient outcomes.
