Qualifications

• Current licensure as a Physical Therapist in Pennsylvania.
• Bachelor's or Master's degree in Physical Therapy from an accredited program.
• Minimum of 1–2 years of experience in a long-term care setting preferred.

Responsibilities

• Assess and evaluate the physical abilities and needs of residents.
• Develop and implement individualized therapy plans to achieve maximum functional independence.
• Conduct physical therapy treatments, including exercises, stretches, and other interventions.
• Monitor and record resident progress and adjust therapy plans as needed.
• Collaborate with doctors, nurses, and occupational therapists to ensure comprehensive care.
• Maintain accurate and complete records of resident treatment and progress.
• Participate in interprofessional rounds and meetings to discuss resident care and goals.
