Job summary

The Springs at Wyandot Trail in Ohio is seeking a licensed Physical Therapist to provide a full range of PT services. You will assess patients, plan treatments, and apply therapeutic interventions in collaboration with an interdisciplinary team.

The role requires a PT degree and state licensure, with 0–1 year of experience preferred. You will educate patients and families, document progress, and support goals for functional recovery in a patient-centered environment.

Qualifications

• Must have a degree in Physical Therapy from an accredited program.
• Must hold and maintain a current, valid state PT licensure.
• Experience: 0-1 years (preferred).

Responsibilities

• Conduct thorough assessments to evaluate patients’ physical conditions, functional limitations, and rehabilitation needs.
• Develop and implement personalized treatment plans, including therapeutic exercises, manual therapy techniques, and modalities to improve patients’ physical function.
• Educate patients and their families on exercise techniques, posture, body mechanics, and strategies to manage their condition effectively.
• Maintain accurate and up-to-date documentation of patient progress, treatment plans, and outcomes in compliance with facility policies and regulatory requirements.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.
