Synchrony Rehab is seeking a Physical Therapist to provide a full range of PT services, including assessments, planning and interventions in an interdisciplinary setting. This role emphasizes patient education, documentation, and collaboration with the care team.

Requirements include a degree in Physical Therapy and a current state PT license. Prior experience is not required beyond 0–1 year, and CPR is preferred.

Qualifications

• Education: Degree in Physical Therapy from an accredited program.
• Experience: 0-1 years in PT or closely related field.

Responsabilités

• Conduct thorough assessments to evaluate patients’ physical conditions and rehab needs.
• Develop and implement personalized treatment plans with exercises, manual therapy, and modalities.
• Educate patients and families on exercise techniques, posture and body mechanics.
• Maintain accurate patient progress documentation compliant with policies and regulations.
• Collaborate with interdisciplinary team to ensure comprehensive care.
• Other duties as assigned.
