FOX Rehabilitation is expanding in Greater Stuart, FL, seeking licensed physical therapists to deliver one-on-one geriatric care in patients’ homes or senior living communities. You’ll evaluate, plan, and deliver evidence-based therapy with autonomy over your schedule, including optional telehealth components to improve access and continuity of care.

Qualifications

• Valid Physical Therapy license in the state(s) of practice or eligibility to obtain
• Basic computer and EMR proficiency
• Current CPR certification

Responsabilités

• Provide one-on-one physical therapy to older adults in home or senior living settings
• Perform evaluations, develop care plans, and deliver evidence-based treatment
• Deliver therapy sessions to improve mobility, independence, and functional outcomes
• Document effectively in FOX EMR in accordance with regulatory standards
• Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff to promote safety and aging in place
• Provide telehealth/virtual care when appropriate to extend access

Responsibilities

• Provide one-on-onephysical therapyto older adults wherever they call home (Privateresidences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)

• Perform evaluations, develop individualized plans of care, and deliverevidence-basedtreatment

• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence

• Document effectively within FOX’s EMR systemin accordance withregulatory standards

Physical Requirements

FOX clinicians must be able to meet the physical demands ofin-homeandcommunity-basedcare, including:

• Exerting medium physical effort, including lifting 25–50lbsoccasionally, 10–25lbsfrequently, and up to 10lbsconstantly

• Performing CPR

• Providing moderate to maximumassistancewith patient transfers and bed mobility

• Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities

• Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model

• Wearing PPE whenrequired
