The Center for Rehab & Nursing at Washington Township is seeking a dedicated Physical Therapist (PT) to join our skilled nursing facility team. You will play a pivotal role in promoting residents' physical well-being and functional recovery through comprehensive therapy plans.

As part of the rehab department, you will conduct assessments, develop individualized plans with clear mobility goals, and ensure accurate Section GG data collection for various assessments.

Aufgaben

• Assessment and Evaluation: Conduct comprehensive physical assessments to determine residents' physical impairments and functional limitations, focusing on mobility, balance, and gait.
• Individualized Care Plans: Develop individualized treatment plans based on assessments, with clear goals for improving residents' mobility and reducing pain.
• Section GG Data Collection: Accurately and consistently collect Section GG data as required for Annual, Quarterly, Falls, and Significant Change assessments.
• Treatment: Administer physical therapy interventions, including exercises, manual techniques, and modalities, to enhance residents' physical function.
• Education: Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conducts remote check-in through remote therapeutic monitoring (RTM) with discharged patients to home or next level of care.
• Documentation: Comfortable and knowledgeable with Point of Service (POS) care delivery and documentation. Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, HMO/Manage care progress updates in EMRs (Net Health/PCC).
• Interdisciplinary Collaboration: Collaborate with the rehab team, including Occupational Therapists and Speech-Language Pathologists, to ensure comprehensive care for residents.
