Base pay range

$30.00/hr - $45.00/hr

JOB TITLE: Physical Therapist JOB CLASSIFICATION: Full time, Part time REPORTS TO: Execuctive Director JOB PURPOSE: Evaluate patient and provide physical therapies for consumers. Supervise therapy assistants, students and volunteers as needed. Write evaluation reports and individualized treatment plans for people served and measure progress on goals. Design, implement and train program staff to implement task for treatment, design treatment activities as a team, lead groups and activities, measure progress and evaluate the program's effectiveness, consumer satisfaction and quality features. Oversee completion of required documentation. Work with physicians on treatment plan and patient needs. Lead projects and handle staff assignments for services provided by the team such as adaptive equipment services. JOB RESPONSIBILITIES: Conduct physical therapy evaluations. Write evaluation reports and treatment plans. Design treatment tasks, implement with consumers and train other staff to assist. Oversee completion of required documentation for physical therapy services. Assist with program development. Participate in team/family conferences. Provide community outings when appropriate (e.g. grocery shopping, recreational activities, fitness center, work trials). Participate in discharge planning. Provide consumer/family education when needed. Recommend adaptive equipment as needed. Participate in fundraising activities. Complete continuing education credits as required to maintain licensure. Maintain clean and orderly environment. Familiarize self with and follows facility and department Policies and Procedures. Follow all personnel policies. Perform other duties as assigned by the supervisor. JOB SPECIFICATIONS EDUCATION/EXPERIENCE: Graduate of an accredited school of physical therapy (bachelor's or master's degree). Missouri license for physical therapy. Experience with brain injury or neurological conditions populations preferred. MANDT crisis intervention certification, CPR certification and first aid training required (these trainings will be provided upon hire along with required agency trainings).
