Job summary

Covenant Living Communities and Services is seeking a physical therapist to provide skilled PT services in a home health setting. You will assess, plan and implement therapy programs, educate families and staff, and participate in discharge planning to support patient recovery.

The role requires a PT license, CPR certification and 2+ years of experience, with a comprehensive benefits package and competitive pay range. Equal opportunity employer.

Responsibilities

• Assessing and evaluating therapeutic, rehabilitative, functional status and participation in the total plan of care.
• Directing physical therapy treatment.
• Initiates physical therapy program and instructs other personnel and/or family /caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.
• May train patient in the use of prosthetic device.
• Participates in discharge planning for patient and in peer consultation process.
• Provides physical therapy consultation to home families/caregivers when indicated and provides Inservice education programs for nursing organization personnel as needed.
• When therapy is only skilled service, instructs, supervises and evaluates home health aide care every two weeks.

Pay Range: $100-110K

Essential Responsibilities

• Assesses for muscle strength, mobility, gait, ROM – potential for rehab.
• Directing physical therapy treatment.
• Initiates physical therapy program and instructs other personnel and/or family /caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.
• May train patient in the use of prosthetic device.
• Participates in discharge planning for patient and in peer consultation process.

Physical Requirements

• Ability to stand, sit, talk, reach, stoop, kneel, and use hands and fingers to operate a computer, telephone, and keyboard.
• Ability to lift, carry, push, and/or pull light to moderate amounts of weight up to 100 lbs.
• Ability to operate various equipment base on department.
• The ability to hear, understand, and distinguish speech and/or other sounds (e.g., machinery alarms, medical codes or alarms. Four hearing requirements listed: In person speech, Telephone, Other remote speech and other sounds.

Clarity Of Vision

• For near visual acuity Clarity of vision at approximately 20 inches or less (i.e., working with small objects or reading small print), including use of computers.
• For far visual acuity, Clarity of vision at 20 feet or more. This is not just the ability to see a person or object, but the ability to recognize features as well.
• For peripheral vision, observing an area that can be seen up and down or to right or left while eyes are fixed on a given point.

Compensation Pay Range

$81,800.00 - $117,097.00 per year

Reasonable Pay Estimate

A reasonable estimate of the pay range for this position is $81,800.00 - $117,097.00 per year. There are numerous factors taken into consideration in determining the actual offered rate of pay, including but not limited to: job-related qualifications, experience, skills, education, geographic location, and consideration of internal and external equity.
