21st Century Rehab, PC is seeking a Physical Therapist for a full-time role in the Central Iowa area. The position offers mentorship, professional development, and a supportive culture focused on one-on-one patient care across hospitals, outpatient clinics, home health, and long-term care settings.

The ideal candidate is a graduate of an accredited PT program, licensed or eligible in Iowa, with a valid driver’s license, and a commitment to growth and teamwork. New grads welcome to apply.

• Graduate of an accredited Physical Therapy program.
• Licensed or eligible for licensure as a Physical Therapist in Iowa.
• Valid driver’s license.

Responsibilities

• Provide high-quality physical therapy assessments and individualized treatment plans.
• Deliver care in hospital outpatient setting.
• Actively participate in patient care planning and interdisciplinary communication.
• Complete required documentation and maintain accurate, timely records.
• Uphold high standards of professionalism, ethics, and accountability.
