Job summary

Renewal Rehab in Niles, Illinois, is seeking a dedicated full-time Physical Therapist to join our team. The role involves conducting detailed evaluations and developing treatment plans for patients, adhering to MD's orders and compliance standards. Candidates must hold a recognized degree in Physical Therapy and possess a valid state license. Qualifications

• Graduate of an accredited university with a recognized degree in Physical Therapy.
• Current license and/or registration as a Physical Therapist in the applicable state.

Responsibilities

• Conduct comprehensive physical therapy evaluations based on MD’s orders.
• Develop detailed treatment plans including goals and modalities.
• Ensure all MD orders are followed for evaluations and treatments.
• Provide comprehensive treatment to patients and modify as necessary.
• Complete all required documentation accurately.

• PT holds a current license and/or registration as a Physical Therapist in‑state as applicable.

Responsibilities

• Physical Therapist provides a comprehensive physical therapy evaluation based on MD’s orders.
• Physical Therapist provides comprehensive treatment plans including long and short‑term goals, frequency, duration, and treatment modalities.
• PT ensures MD orders are obtained for evaluations, treatments, re‑certifications, and discharges.
• PT provides comprehensive treatment to patients utilizing modalities and modify patient treatment as indicated while adhering to precautions.
• Physical Therapist completes all required documentation.
